Livescan - What to Bring

To ensure rapid processing, please be sure to bring all of the items listed below, when you arrive for your Live Scan Fingerprinting.

• Three (3) copies of a completed Request for Live Scan Service form.

Request for Live Scan Service Form 

The request for Live Scan Service form is a government form, identified at the top with “REQUEST FOR LIVE SCAN SERVICE-Applicant Submission” or with the code “BCII 8016”, which is located on the bottom left corner. This form must be completed prior to being fingerprinted. If you do not have one, please contact the agency or person who is requesting you to be fingerprinted, as we do not supply the form at our facility.

• A valid government-issued picture ID, such as a driver’s license or passport. 
• Payment for the Live Scan service in the form of exact cash, check, Visa, Mastercard or Discover credit card.
